That awkward moment where you werewolf-murder every single living creature in a particularly hard dungeon, then realize three of the people you killed were your follower and two hired mercenaries.
That awkward moment when you try to use Unrelenting Force in Windhelm to get a "Letter from a Friend", and you have Storm Call equipped. (for you noobs who don't know what Storm Call is, it's a shout that summons a storm cloud that zaps everyone in the area with lightning, even civilians and guards.)
That awkward moment when on your first playthrough ever you kill Astrid only to find out later that she's the leader of the dark brotherhood afterwards and she had a heap of cool missions waiting for you
...you hear a male thrall groaning in a female voice.
...you're going through a bandit hideout and discover a blood-covered bed. There aren't any bodies or bones around and there's no evidence to support the notion of foul play. All you can figure is that some poor sod had a case of bleeding stomach ulcers coupled with a night of explosive diarrhea. You come away convinced that this is absolutely the answer.

The Awkward moment when an Ancient Dragon attacks Solitude and it kills all the guards. (Happened to me, every single guard in Castle Dour was dead and there were bodies all over the place. Later they went to Hall of the dead and there were shields and swords left. Got me a good amount of coin)
