Champion of Grottos and Gremlins
Something weird happened today. I was playing my Argonian thief, roleplaying his journey from Windhelm to Whiterun, where Belethor is my fence. I was carrying literally 300 weight in items. 1 more gem or scroll and I would no longer be able to run. So I am ignoring all the profit chances that get past me in the journey, focusing only in the destination...

I get to the plains unharmed, without a single random event that far, but then... it happened. My character screamed in pain as I lost 25% of my health. At first, I thought it was some wolf that got me from behind, so I started spinning around like a retard for 15 seconds, looking for whoever had dared to attack me. (BTW I wouldn't be surprised that a wolf had taken me 25% HP because that character was low level and wore clothes instead of armor). No, no wolves nearby. My compass doesn't show anything near me. The only movement in the plains is of a butterfly and a mammoth in the distance. I even started sneaking, and nothing was watching me.

Then, I thought I could have walked over an object and lost some health, although 25% seemed a bit too extreme, so I spend a few more seconds looking at the ground like an idiot, finding nothing once again. By the time I was done searching for the cause of my health loss, I had regenerated my entire HP back, and decided to carry on...

Did I just have a heart attack?! Anyone else had this?
 
actually I had this happen to me just recently xD boy did it scare me! And what was even more strange is I was also heading from Windhelm :D my character was walking on the road and suddenly she screams as if she just had a painful orgasm. I almost died of a heart attack, and literally searched everywhere for the cause for a full 30 minutes! I still don't understand it xD

Sounds like the cause was a trap below you in the ground. The engine regularly misplaces objects. Nothing like killing a spider to see it disappear into the ground so you can't loot it.

It could have been one of those deer/cow skulls. Those things can be quite damaging if you have low HP, and running over them can knock them quite a distance, which may be why you didn't find it.

My guess is you tripped over one of those non-interactive havoc objects, like the animal skulls that fly around the player homes. They do damage when you step on them, but you can never get them out of your way. That's my guess anyway.
 
I had that happen to my Orc character on her way up the Throat of the World. Damaged her follower too. I spun around for a good 10 minutes or so but found nothing. It happened a few times actually, she'd scream and her health would go down like 5 percent. I thought it was an ice wraith because they're hard to see but no red dot. Go figure. Interestingly she swung her axe around a bit and it stopped though. Odd.

I'm thirding (is that even a word?) the animal skull theory.

I also had something similar(ish) happen in the Rift. I was in werewolf form (full moon) when I came across one of those Imperial prisoner transfer parties, near Sarethi Farm. After I'd killed all the Imps (and feared the prisoner & patrol dog), I was attacked by invisible things. Turning on predator vision revealed three dog-sized enemies, however the enemy health bar said "Imperial Soldier". They stayed invisible after I finally managed to kill them, and since werewolves cannot loot (except to harvest blood, oddly enough) I never did find out what or who they were.
