Did you place the ESP. file in Morrowind's Data File directory?
I just put it under "Morrowind" in my files...
In the Morrowind files, there should be a folder called 'Data Files'. Is there?

Ok. Now run Morrowind.
I'm running morrowind, I clicked "data files", and accurate attack is nowhere to be seen. All I see is morrowind.esm, bloodmoon.esm, tribunal.esm, and all of my saves with the dumb names I gave them
Odd. Try playing the game and attacking something, to see if it hits every time.
Not all of the swings hit.

Sorry, can't help you there. It might be a problem on your end. I'll see if I can't find a way to fix it by looking at my files.

Can you find your Morrowind.ini file in the Morrowind folder?
