    same issue here, my sneaky assassin build sometimes missed a clear shot, even if I aim perfectly on the target's body. doesn't happen very often, though. it fixed by itself. and the bug starts again sometime.

    Previous Save File Being Deleted

    save your game as "New Save" since Auto Save can overwrite your previous auto saves up to three slots. If you play on PC you can also do a quick save by pressing F5. (F9 to quick load). remember to save your game as "New Save" before you start another game. :)
