TM5000 here and I usually play games years after they are released.

I also have some favorites I have been playing for years and will probably never stop.

I first played Neverwinter Nights around 2007, S.T.A.L.K.E.R. around 2012, Fallout 3 around 2013...

Now I am installing Skyrim for the first time!

My knowledge of RPGs is old school DnD (1st Edition) and NWN (3.5 ish)

I understand there is a system in Skyrim simlar to Fallout 3 with perks which I am somewhat familiar with, but I will still welcome any newbie advice.

What should I start with?
 
Use whatever works! That's how Skyrim's levelling system works. Your skills improve with use. Once you improve any of your skills a certain number of times, your character level will increase, allowing you to improve either one of your health, stamina, or magicka pools by 10 points, and granting you a perk point to invest in any skill tree. You can only invest perks into a skill tree if you've levelled that skill up enough, though, so you'll find yourself naturally specialising.

So if you find yourself most comfortable fighting with a sword and shield, your single-handed weapons and block skills will level up. Then you can invest perks into them to make them even more effective.

Play around, and see what you prefer. Whatever you use most is what will level up the most. There are no classes in Skyrim - you can mix and match skills.

Skyrim really reminds me of Fallout3 and I seem to be successfully making a good character.

I got in over my head after returning a Dragonstone (I think) when I found myself volunteered to fight a Dragon.

Too soon for this toon! I went to a previous save.

So Lydia is becoming a pain - she gets lost all the time causing me to go back and look for her.

In Whiterun I decided to tell her we should part ways, and she walks into my house! Are we going steady or something? I actually had my eye Aela the Huntress.

Also, the next guard who asks me if I fetch the mead for the Companions may get my battleaxe in his forehead. I imagine that will alter my rep in the city.
