I swear this is totally true. On one of my earliest playthroughs I was running around the map looking for anything and everything. I had recently fought a bunch of ghosts with the Companions and was handing around just outside of Whiterun. I faint blue light caught my attention, I simply thought it was a ghost I missed (stranger things happen) so I readied my blades and waited. As the ghost drew closer I noticed it was on an erethral horse. Strange, but I was fairly new to the game. The ghost ran up and didn't show any indication of stopping. I sidestepped and swung my swords but missed when I was the character name. I thought I was crazy so I sprinted after and overtook the horse. Sure enough, the rider had no head and the white title read "Headless Horseman." I have never seen it again. Has anyone else seen it? Is there a reason why he's in the game?
 
I've seen him once. I think it was somewhere out in the countryside near Rorikstead. It was pretty cool because I was out of town and playing the game on an HDTV that my hotel room had - I have a tiny oldschool analog TV in my room 'cause that's all I have room for (I live in an RV so limited space, man). So my first time seeing him was in high def. I didn't know what he was either at the time and I did the same thing as you - tried to attack him but both spells and swords went straight through him. I honestly thought it was some bugged out NPC or quest giver, so I made a new hard save and looked it up. Seems it's just some kind of Easter egg Bethesda put in. I have heard rumors that if you get that potion that makes you look like a ghost - forgot what it's called now, it's in that barrow in the town underneath the Throat of the World, I know that - and take it then talk to him, there's some sort of unmarked quest you can do. Others say that by taking the potion you can just talk to him a little. Haven't tried it myself but I mean to eventually. :)

How did you figure that out? I've never played Morrowind, so I wouldn't get that reference. However, someone made mention of a city beneath the throat of the world? Is that true for the vanilla game without any DLC add-ons?

There is no city under the Throat of the World. However, there are massive Dwemer ruins that span across most of Skyrim that are underneath the mountain range from east to west.
