I've become so used to encountering enemies out on the road that my default reaction has been shoot first and ask questions later.
#1. I was sneaking up to the entrance of a particular cave and I saw a tall blond fellow standing there who looked a lot like a bandit. Okay, down him with my cross bow because better safe than sorry. I loot the body and find out he's actually Golldir, a innocent man too scared to enter the tomb and waiting to give me a quest to rescue his aunt. Oh well. Take his keys and clear out the tomb. Kill the boss Vals Veran and get a 'text message' stating I've completed the quest, it just doesn't show up on my quest log at all.
#2. I bump into Angi in her mountain camp, have a convo and shoot at her targets. Level up the archery skill twice first round and fail the next one: 8 second time challenge. She's like "oh, better luck next time," and walks away. I turn to talk to her and ask for a rematch, and accidently shoot her dead with an arrow. Crap! It was an accident, I swear. Oh well. You were kinda rude anyway and on the run from the imperials, which I actually am a soldier with so I did everyone a favor.

How do you "accidentally" shoot someone in the back?

Bumping into Angi isn't exactly easy since she lives in a very isolated area wayyyyy off the beaten path. I'm a veteran at this game and sometimes I have problems finding her.

I still haven't completed the last challenge...
I always kill any stranger who carries a weapon visible from a distance. I didn't mean to piss Farkas off, but immortals are immortals, right? Died quicker than a Bunny in Blackreach.

It wasn't planned. More of a random encounter while traversing the wilderness. If you're actively looking for her it's probably harder, ironically.

Most of the time, no matter what character I'm playing, I'm calm, cool and collected that is until I spot the Thalmor along the road. Within seconds, I'm raging and all of my focus is centered on taking them out as quickly as possible. I absolutely hate the stuck up bastards and that probably explains why I keep a bounty on my head with any character I play.

I once killed an innocent hunter, but I maintain having a woman dressed in studded armour shooting arrows in your direction while running towards you screaming: "THIS IS THE END FOR YOU!" is very easy to mistake for a bandit, especially if you never spotted the bunny or fox she was really after somewhere behind you.

It depends a bit on which kind of character I'm playing. If its a good person I'll make sure I only attack evil people i meet out in the wild. If I'm playing an evil one I kill first and ask questions later.

That cave where there's a guy in the first room killing spiders... Happened again last night. I see a spider so I pull my bow, draw it back, but then the adventurer kills the spider and speaks to me. As soon as he engages me in dialog, the bow releases it's arrow automatically- right into the dude's chest! OOps..!

I too have one-shotted the frightened woman. Out nowhere she came running and shrieking at me and I instinctively let fly.

Your not the companion I'm looking for.... To trigger happy ;)

Simple rule I use is never attack first. So many quests break if you do.
