Xbox Live disconnecting.
If it was me Lee, i would run a constant ping on my xbox (from a pc or laptop), when you disconnect from xbox live check your pings.. they should drop too if it is a network issue. If it is a internet issue, you can run a constant ping on www.yahoo.com. Same thing, if xbox live drops check the... -

food for thought: your follower can only die if you kill her or area kill.. when your follower gets low on health they will drop to a knee to recharge. Now while your follower is down if you hit them with an arrow/sword/spell/fire etc they will die.. Or if they go down and a dragon is burning...- tragik

yes, you can go find another follower. there are many, if you want to "buy" one you can find another in the drunken archer or what ever store in whiterun.. She will be sitting there at a table for 500 coin. there are many other followers you can get as well.- tragik

Just for testing.. turn off all auto saving (menu and auto x minutes).. I have read that it help with the PS3 version.. again I do not have any of these issues so just trying to give some help.- tragik

another dumb question, is your game loaded onto the hard drive or playing from disk? are you saving to the "cloud" or locally? do you have the auto save on menu and auto save ever x minutes turned on?- tragik

PC Moonshade and Nirnroot
Nightshade is all over if you go by the swamps, between riften and windhelm (closer to riften).. they are all over the place (not the waters edge). 3v3r is correct on the ninroots. the main thing I do is listen for them, they will make an annoying sound. Also if you go by riften in the water...- tragik

Triangles?
If the triangles are on the "right" ( and point up) of the device that means it is better weapon/armor than what you have on/equipped. If the triangle are on the "left" (and point to the name) then that is a "favorite" item..- tragik
